Yumi Taylor, MD (She/Her)
Founder, Head of Hōtto
Dr. Yumi has been caring for aging adults, first as a caregiver in a community group home for individuals with intellectual disability prior to medical school, then in primary care internal medicine for 15 years. Departing from the usual Ivy academic pathway of specialty care, she moved into primary care private practice in her rural hometown. She traveled to see her panel of patients in the hospital, at their home, in the nursing home, free clinic, Medicaid clinic, and in the office as was common for small town docs. She returned to academics for 2 years of executive training in healthcare technology management. From there she joined a large healthcare system, innovation center, & health plan to build a modern, flexible primary care model that delivered care primarily through messaging with a team of physicians, coaches, and advanced practice nurses.
What was expected to be a model primarily for younger, healthier patients, turned out to be sought by family caregivers of aging loved ones losing independence. Identifying dementia, engaging the family and other caregivers, providing flexible communication, attending to medication details, owning the quarterback role, and supporting creative problem solving are where Dr. Yumi and her team leaned in.
Dr. Yumi established Hōtto to provide a wrap-around clinical and social support model for those with dementia and their family caregivers.
